Trailblazer Community Care comes with rich nursing and lifestyle services of over 60 years' experience within the team. We are led by Registered Nurses and Senior care workers. Care plans are person-oriented, tailor made with the participant and key person. Together we collaborate in the best interest of the participant.
Our nursing team has experience in complex wound management, catheter management, diabetic care, palliative care and medication administration. We do depo shot, prompt catheter changes in simple blocks/complication. We also do continence assessments.
Support staff work in conjunction with the nurses. We have experience in high intensity cares, community access, light duties. We are a multi-cultured team and serve all persons of all backgrounds. We are LGBTI friendly. We offer an unmatched continuity of cares and do not send a new person to you every other week. Our admin staff are hands on and will know you in person and understand your care needs. We are flexible and adjust our ways to your care needs.

We are a small business that is well run by experienced RNs with combined 30 plus years in the health sector. Our team of support workers are highly motivated to know you and support you in your journey of life. We are a registered identified NDIS company and respect diversity of our cultures within the communities we serve. Our participants are heavily involved in the care planning and how supports should be structured in tailor making the care routine. We are a small provider, and all our participants are well known by admin and getting messages or making changes to your day or routine is quickly actioned. Our staff are reliable as such we offer continuity of care given if happy and achieving expectations your support staff will be supporting you regularly as opposed to meeting a new person every week or so. Our referral pathway is simple and the turnaround time to start supports is generally 2-7 days depending on the contact hours. We offer a no obligations first meeting at your convenience.
Scope of Practice:
Nursing team
We are experienced in nursing services: mental health nursing, catheter cares, wound care, medication administration, diabetes management, stoma cares, training support workers.
Support services team
Our support workers are trained in high intensity cares including : bowel therapy, hoisting, medication assistance, stoma cares, catheter cares, pressure injury cares. We also do community access, domestic assistance, mental health supports.
Accommodation
We also do STAs, ILO AND SILs. We have a few properties that are not SDAs but accessible in the Logan region.
