Support Coordination Lead

Chosen Family are seeking a passionate and highly experienced and driven support coordination lead who will create, grow and lead the support coordination department. We are seeking someone who is great at networking and will build the case load and manage for chosen family. The desire is to eventually lead a team of coordinators who are passionate about those we serve in our community. As KPI’ s and targets are met, bonuses will be on offer. This role as a NDIS Support Coordinator focuses on assisting participants in navigating and maximizing their National Disability Insurance Scheme (NDIS) plans. You'll collaborate closely with participants to understand their needs, connect them with appropriate services, and empower them to achieve their goals. This position requires strong communication skills, deep knowledge of the NDIS framework, and a commitment to enhancing the quality of life for individuals with disabilities.
Key Responsibilities:
- Create and lead the support coordination of chosen family.
- Work closely with NDIS participants to implement and adjust their support plans, ensuring alignment with their goals and funding.
- Visit participants in their homes to assess needs and provide personalized support.
- Facilitate connections between participants and service providers, ensuring they have access to necessary supports.
- Monitor progress towards NDIS plan goals and submit required reports to the NDIA.
- Collaborate with community and government agencies to enhance participant outcomes and resolve issues.
- Foster strong relationships with care teams, informal supports, and legal services to support participants effectively.
- Demonstrate excellent interpersonal and communication skills, both written and verbal.
- Manage administrative tasks effectively, including organizational and report writing duties.
Candidate Requirements:
- MUST have co-ordination experience.
- Relevant qualifications in fields such as Mental Health, Psychology, Social Work, or related human services.
- Display a strong understanding of compliance responsibilities.
- Demonstrated experience in supporting individuals with diverse needs like Mental Health, Autism, and Intellectual Disability and individuals from diverse and lgbtqia+ community
- Comprehensive understanding of NDIS plans and processes.
- Ability to work autonomously and remotely, with strong time management skills.
- Valid driver's license and access to a vehicle, along with necessary background checks.
- Experience in crisis intervention, AOD issues, homelessness, or justice-related work is beneficial.
