Assistance From Live-In Carer

A live-in carer delivers supports in daily living activities, including self-care and household tasks. This support is delivered around the clock.

STA And Assistance (Inc. Respite)

24/7 temporary accommodation and supports, including meals and activities, for up to 14 days per stay and 28 days per year.

Assistance In A Shared Living Arrangement

Support with daily tasks such as personal care, domestic chores and community access within a shared living arrangement (SIL). Does not include costs for rent and bills.

Assistance In Individual Living Arrangement For Person With Complex Needs

Daily living supports, such as personal care, delivered by experienced support workers for participants with complex needs. These are delivered in your own living arrangement.

Assistance In Living Arrangements (Host Family/Alternative Family Situation)

Supports delivered by a host or alternative family in their home. Supports range from daily tasks to complex care needs, depending on individual requirements.

Assistance with Personal Domestic Activities

Support delivered by a support worker to maintain their home environment such as cleaning, meal preparations, dishwashing, laundry and gardening.

Assistance With Self-Care Activities

Support with everyday domestic tasks, delivered by support workers, including medication management, meal preparation, personal grooming, toileting and showering.

Assistance With Self-Care Activities - High Intensity

Significant support with everyday tasks, such as eating and grooming, delivered by skilled or experienced support workers for individuals with high intensity needs or complex disabilities.

Assistance with the cost of the preparation and delivery of meals

Meal preparation and delivery including prepared meals and meal kit delivery services.

Capacity Building and Training in Self-Management and Plan Management

Assistance in understanding the requirements of self-managing NDIS plan funding, delivered by qualified professionals. This support builds capacity by teaching budgeting and administration duties required of self-managed participants.

Community Nursing Care - Clinical Nurse

Delivery of health supports in a specialised area, by a Clinical Nurse, who has completed additional studies and attained experience in the specific field, such as chronic wound management, diabetes management and incontinence management.

Community Nursing Care - Clinical Nurse Consultant

Delivery of advanced health supports, by a Clinical Nurse Consultant, including conducting medical assessments, coordination of care and providing expert advice and education to participants.

Community Nursing Care - Enrolled Nurse

Delivery of health supports, by an Enrolled Nurse, to oversee various health conditions. This includes medication administration and health monitoring within participants' homes or community settings under the direct or indirect supervision of a Registered Nurse.

Community Nursing Care - Nurse Practitioner

Delivery of comprehensive healthcare needs, by a Nurse Practitioner, including diagnosis and treatment for less complex health conditions, conducting medical assessments and coordination of care.

Community Nursing Care - Registered Nurse

Delivery of health supports, by a Registered Nurse, to participants with complex health care needs. This includes developing care plans, conducting assessments, medication administration, wound care, chronic disease management, urinary catheters, complex bowel care, PEG feeding, and subcutaneous injections.

Household Cleaning and Yard Maintenance

Assistance with general housekeeping and garden care tasks such as cleaning, mowing lawns, weeding, and minor repairs. This is delivered by skilled cleaners, gardeners and handymen.

On-Call Overnight Monitoring-Off Site or Onsite

Overnight on-call assistance or supervision with daily living tasks, personal care, emergency response, or medication administration, including one hour of direct support. Both off-site and on-site options are available.

Physiologist

Assessments, therapy and training supports delivered by a physiologist. Services support individuals with coping strategies, mental health rehabilitation and development of social skills.

Specialised Home Based Assistance For A Child

Supports for a child when informal supports are unable to undertake their caring duties. Delivered by professionals, supports include help with daily routines, skill-building activities and behaviour management.

Speech Pathologist

Personalised assessment, therapeutic interventions, and articulation training by a speech pathologist. Services may include articulation training, language development strategies, social communication skill building and swallowing dysfunction therapy.

Disability-Related Health Consumables

Everyday use items for disability-related health concerns such as wound care supplies, incontinence aids, and nutritional supplements. Does not include assistive technology items.

HEN Equipment & Feed

Home Enteral Nutrition (HEN) feeding products and Percutaneous Endoscopic Gastrostomy (PEG) equipment for alternative feeding. This includes consumables such as feeding tubes and feed bags or bottles and PEG equipment.

Incontinence Products For Adult

Washable and single-use incontinence/continence products for adults such as catheters, absorbent pull ups or briefs, inserts and pads.

Incontinence Products For Children

Washable and single-use incontinence/continence products for children such as reusable nappies, undergarments, and bedding products.

Assistance in Supported Independent Living (SIL) - High Intensity

24/7 supports delivered in an accommodation setting to help or supervise with daily tasks, like personal care or cooking meals. This is for participants with high intensity support needs requiring a significant level of constant support at home. SIL is usually delivered to a shared living arrangement by skilled support workers. This does not fund rent or other day-to-day expenses.

Assistance in Supported Independent Living (SIL) - Standard

24/7 supports delivered in an accommodation setting to help or supervise with daily tasks, like personal care or cooking meals, for participants with higher support needs requiring constant support at home. SIL is usually delivered to a shared living arrangement. This does not fund rent or other day-to-day expenses.

Individual Living Options (ILO)

A package of primary and supplementary home and living supports, that can be used flexibly in combination with formal and informal supports. Can include support to explore living options, personal care and support, skill development and independent living support.

Medium Term Accommodation (MTA)

Temporary accommodation, up to 90 days, for individuals awaiting long-term housing. Does not include supports delivered by a support worker, meals and activities.

Transport

Support with traveling to work or other places in the community.
